Supporting the elderly, children, young people, families and carers, they run programs to support the community such as before and after school care, casework, food relief programs, amazing classes on topics such as self care & wellbeing, respite care, group outings and so much more!

The elderly support of this organisation is growing and due to growth, this organisation are now looking for a Clinical Case Manager - (Home Care Packages) to join their awesome team!

Role & Team

As Clinical Case Manager, you will be managing a small case load of home care packages clients with complex clinical needs to support them in being able to stay at home. You will also have the opportunity to develop the clinical oversight and governance, being able to grow your clinical governance knowledge through having access to a clinical specialist board member. Your role will be a mix of working from home, the office and visiting your clients in the South East/East Sydney area.

You will be able to be out and about, with no day the same as your role will involve home visits to assess & meet your clients, conducting referrals, budget management, reviews and goal planning & writing clients care plans to help clients live their best lives. Your role will include building strong relationships both externally & internally, communicating with care workers, allied health, providers, families & clients.

You will be working in a vibrant and passionate team of 4 - with the support of a great manager plus your colleagues.

  • You are passionate about helping the aged community & working to support clients live their best lives!!
  • You are experienced with Home Care Packages 
  • You are an AHPRA Registered Nurse
