This very well respected Not For Profit organisation have been around for 50+ years, providing multi-disciplinary services & support to the community across aged care, disability & settlement services, as well as bringing communities together.

The in-home aged care division of the organisation provides services to 300+ clients across Home Care Packages and CHSP with a team of passionate people who strive to support aged persons to stay at home.

Based in Sydney's south-east you will be offered working from home flexibility, as well as flexibility to choose your working hours between part time or full time.

Role 

Your role will be to provide support to aged persons in the South East of Sydney, managing levels 1-4 home care packages, coordinating with your clients on a regular basis via in-home visits, (& video & phone communication) to help them meet their goals and lifestyle, health & financial requirements through; assessments, re-assessments, budget planning & negotiation, providing quality care, liaising with/ managing support workers and referrals to external stakeholders such as allied health.

Your role will vary between being in the office, at home and visiting your clients.

You will be fulfilling your love for helping people and will be supporting the Greek community, using your bi/multi-lingual skills to provide culturally & linguistically sensitive support.

Team

As Case Manager, you will be joining a vibrant, experienced & passionate team of 5 case managers and a wider team of 70 staff.

You will be provided with the support of a Roster Coordinator which means you will not be carrying out rostering, as well as having the clinical support of RN's in the organisation.
