Job Description


This leading national community care organisation provide in-home Disability & Aged Care services as well as Clinical support. With an employee force of nearly 2000, they have a well knows presence with experienced staff.

This organisation provide Government funded aged and disability care and due to growth, are now looking for an experienced Care Manager to support and manage services.


Reporting to the Disability Manager, you will be joining a motivated team to support a case load of private disability customers, building care plans and managing the ongoing support and assessments, ensuring customer satisfaction, high levels of care and ensuring packages and programs are managed within budget allocation.

This will involve the identification of new customers and proactively ensuring that the level of customer care is in line with agreements, contracts and that you are providing a person-centred support approach. You will also be responsible for ensuring customer administration, systems & procedures  are followed consistently.

As well as managing your clients needs and care plans, you will be facilitating the transition of customers in to the NDIS environment.

About You
  • Experience in Disability 
  • Clinical experience - Registered Nurse qualification
  • Case or Program Management - ability to balance multiple clients
  • A tertiary qualification in a relevant field e.g. social work, allied health, nursing, case management, Disability
  • Experience managing client budgets 
  • Understanding of the NDIS environment
  • Happy to travel throughout Sydney as well as some inter-state travel
  • You love working in a growing environment and are able to adapt to change as well as thrive working in a fast paced environment
  • Strong ability to work with internal & external stakeholders such as Government agencies and other teams
  • Business development acumen 
  • Current Driver’s License & police check (or the willingness to obtain one)
